User Forms

The Administrative Information Services (AIS) User Forms provide the administrative computing community with an electronic means of requesting various administrative computing services. The forms are available for submission 24-hours-a-day, seven days a week.

The time involved for a request to be completed will vary depending on the type of service being requested.
