AIS Systems Access Request

If you are an Access and Security Representative (ASR) for your department, you can print the Systems Access Request Form below to request user access to systems, such as IBIS, ISIS, and TSO. Once the form is completed with the appropriate signatures, mail it to the Administrative Information Services Security Office at:

University Park, PA 16802

Please print this two-page form onto one piece of paper using the duplexing print option. This option can be selected under print properties. Check with your local tech support or operating system help files if you are unsure how to do this. The procedure varies for different operating systems.
