AIS Systems Access Request If you are an Access Security Representative for your department, you can print the Systems Access Form to request user access to systems like IBIS, ISIS, TSO, etc.  Once the form is completed with the appropriate signatures, mail it to the Administrative Information Services Security office at 24 Shields Building, University Park. Note:  Adobe Reader is required to view and print this document. Download free Adobe Reader .  To clear form fields when doing multiple forms, open a new browser window for each new form. Please print this two-page form onto one piece of paper using the duplexing print option.  This option can be selected under print properties.  Check with your local tech support or operating system help files if you are unsure how to do this.  The procedure varies for different operating systems.
